 Residential Services

Direct Support Workers:     

 

Actively provides direct services to persons with developmental disabilities in their home and community settings; works collaboratively with the support team to develop and implement individual plans; provides a direct connection for active community involvement. Requires HS diploma & valid MA driver’s license.

 

Assistant Program Directors:

 

Assist with the daily operation of the program and actively provides: direct support to persons served; case management, service planning & implementation of ISP, staff direction, and required programmatic documentation. Requires minimum of 6 months experience in above areas, HS diploma, & valid MA driver’s license. 

 

Program Director:

                   Directly responsible for the daily management & operations of assigned program including staff supervision & training; development of the ISP; healthcare coordination; money, medication, & schedule management; oversee and monitor  program for service quality.  Requires BA, minimum of 1 year of above experience, valid MA driver’s license & vehicle.

Program Coordinator:

          Directly responsible for the daily oversight & management of assigned programs including staff supervision & training; development of the ISP; healthcare coordination; money, medication, & schedule management; oversee and monitor  program for service quality.  Requires BA, minimum of 3 years of related experience, valid MA driver’s license & vehicle.

 

Residential Supervisors:

        Lead direct, & manage multiple programs. Provide supervision & direction to assigned Program Directors & Program Coordinators. Oversee and monitor all programs for service quality; coordinate & provide case management services; facilitate the ISP; and create innovative opportunities, programs and services for the people we serve. Requires BA, minimum of 5 years of progressive management experience, valid MA driver’s license & vehicle.

 

Individual Supports:

IS Program Coordinator:      

Provide case a management, case coordination, and community-based outreach to people with developmental disabilities & mental retardation who are living independently.  Actively provide direct supports; assist in expanding social networks; participate in service planning & implementation of ISP; etc.  BA and minimum of two year’s relevant case management experience. Valid MA driver’s license, access to a vehicle and ability to work a flexible schedule required. 

IS Program Manager:

 

Provide direct supervision, training & support to assigned IS Program Coordinators.  Actively provide direct supports, coordinate and manage the ISP process, provide case coordination including healthcare coordination; money, medication, and schedule management.  Oversee and monitor  program for service quality.  Requires BA, minimum of 4 years of related experience, valid MA driver’s license, access to a vehicle, and ability to work a flexible schedule required. 

Corporate Guardianship Program

Guardian Representative:     

 

Provide advocacy service on behalf of persons served.  Participate in the development of the ISP, monitor services for quality and relevance to the needs of the person served, schedule quarterly home visits, and actively participate in ensuring the desires, wishes, & needs of the person served are addressed.  Requires BA and a minimum of 10 years of progressive experience supporting people with mental retardation, valid driver’s license & vehicle.  Per Diem hourly rate to $25/hr.
